Fill out the Online Booking Application,
sign it, and fax back to Sanctuary Spa at
713.622.0934. A Spa Representative will
contact you with your User ID & Password.
This method could take up to 48 Hours.

Appointment Request Form
The Appointment Request Form will help you
request an appointment. Once you have
submitted your request for an appointment, a
Spa Representative will contact you to
confirm your service and time and you will
be given your User ID & Password for future
bookings. This method could take several
hours, depending on when you send in your
application, as well as your availability
when we call.
